The code was not detecting that the cofactor was set up correctly
if OSSL_PKEY_PARAM_USE_COFACTOR_ECDH was set, resulting in an incorrect
FIPS indicator error being triggered.
Added a test for all possible combinations of a EVP_PKEY setting
OSSL_PKEY_PARAM_USE_COFACTOR_ECDH and the derive context setting
OSSL_EXCHANGE_PARAM_EC_ECDH_COFACTOR_MODE.
This only affects the B & K curves (which have a cofactor that is not 1).
Bug reported by @abkarcher
Testing this properly, also detected a memory leak of privk when the
FIPS indicator error was triggered (in the case where mode = 0 and
use_cofactor was 1).

The code to handle the cipher operation was already in the provider.
It just needed a OSSL_PARAM in order to set this into the algorithm.
EVP_CIPHER_CTX_set_flags() has been modified to pass the OSSL_PARAM.

The existing names such as EVP_PKEY_param_fromdata_settable were a bit
confusing since the 'param' referred to key params not OSSL_PARAM. To simplify
the interface a 'selection' parameter will be passed instead. The
changes are:
(1) EVP_PKEY_fromdata_init() replaces both EVP_PKEY_key_fromdata_init() and EVP_PKEY_param_fromdata_init().
(2) EVP_PKEY_fromdata() has an additional selection parameter.
(3) EVP_PKEY_fromdata_settable() replaces EVP_PKEY_key_fromdata_settable() and EVP_PKEY_param_fromdata_settable().
EVP_PKEY_fromdata_settable() also uses a selection parameter.

Many of the new types introduced by OpenSSL 3.0 have an OSSL_ prefix,
e.g., OSSL_CALLBACK, OSSL_PARAM, OSSL_ALGORITHM, OSSL_SERIALIZER.
The OPENSSL_CTX type stands out a little by using a different prefix.
For consistency reasons, this type is renamed to OSSL_LIB_CTX.

As discussed in the issue, supporting the set0-like semantics long-term is not necessarily desirable, although necessary for short-term compatibility concerns. So I've deprecated the original method and added an equivalent that is explicitly labelled as set1.
I tried to audit existing usages of the (now-deprecated) API and update them to use set1 if that appeared to align with their expectations.

The user can set up a self test callback that should be activated when a keygen operation (e.g ec) occurs for the fips module.
The callback information is stored inside the applications library context, but this was not being triggered since the
library context used for the key generation operation was the internal library context used by the fips module (which is not
the same as the application's library context). During the keygen operation the OSSL_SELF_TEST_get_callback() function is used
to retrieve the callback info.
By having a seperate copy of OSSL_SELF_TEST_get_callback() for the fips module we can ensure that the parent library context
is used instead.
The core OSSL_SELF_TEST_get_callback() function pointer is passed across the boundary during the fips modules entry point
such that the fips version of the function can call it after changing the libctx.

For FIPS validation purposes - Automated Cryptographic Validation Protocol (ACVP) tests need to be
performed. (See https://github.com/usnistgov/ACVP). These tests are very similiar to the old CAVS tests.
This PR uses a hardwired subset of these test vectors to perform similiar operations,
to show the usage and prove that the API's are able to perform the required operations.
It may also help with communication with the lab (i.e- The lab could add a test here to show
a unworking use case - which we can then address).
The EVP layer performs these tests instead of calling lower level API's
as was done in the old FOM.
Some of these tests require access to internals that are not normally allowed/required.
The config option 'acvp_tests' (enabled by default) has been added so that this
access may be removed.
The mechanism has been implemented as additional OSSL_PARAM values that can be set and get.
A callback mechanism did not seem to add any additional benefit.
These params will not be added to the gettables lists.
